Organizations wishing to enroll in our bronze level membership do not require a formal contract. The enrollment form can be downloaded and fax or mailed to our office. Upon enrollment, the organization’s representative (as noted on the enrollment form) will be contacted to receive detailed information on how to access the various services.

Silver & Gold Level Membership
Organizations wishing to enroll in our silver level or gold level membership require a formal contract with CIHQ. Please click on the link below and provide the information requested. A representative from CIHQ will review your submittal and contact you.
